Secondment post for 20 months

An exciting opportunity has arisen to support the development of Asthma services for children and young people across the Shropshire, Telford and Wrekin ICS. The STW ICS has been successful in gaining pilot funding to develop an Integrated Care System for Children and Young People’s Asthma. As one of only seven pilots across the country, we are seeking to recruit a highly specialised paediatric asthma practitioner.

This pilot offers you the chance of leading service innovation and developing good practice to be shared across the country. The postholder will work closely with the ICS clinical Lead for CYP Asthma to help meet the deliverables of the recently published National Bundle of Care for CYP with Asthma. The national asthma bundle was released earlier this year and this role will be pivotal in driving it forward locally.

Main duties of the job

One of the main roles will be to work with the Clinical and GP ICS leads to establish two diagnostic hubs based within two PCN’s within Shropshire and Telford and Wrekin.

This role will be based within the ICS and work across the communities of the ICS, taking a strategic role in the implementation of asthma improvements. They will develop partnerships across the ICS both vertically through healthcare and horizontally with other arm’s length bodies.

Detailed job description and main responsibilities

The post holder will establish relationships through primary, secondary, and tertiary care to ensure high quality of care throughout the system. They will work with the clinical lead to establish diagnostic pathways for CYP with suspected asthma.  The post holder will help to identify areas where asthma outcomes are poorest and establish the reasons for this, working with teams to improve access to CYP asthma training to ensure all healthcare professionals that come into contact with a CYP with asthma are trained to the appropriate level. They will liaise with education authorities to promote Asthma Friendly Schools and work with social services and public health bodies to improve communication and support for school nurses, health visitors, social workers, CYP with asthma and their families.
