An exciting opportunity has arisen for an enthusiastic and forward-thinking Band 6 sister to join our team for a 6-month secondment in Kidney Transplant services 

The Renal  Transplant nursing team at SATH are responsible for educating renal patients and families about their transplant options, coordinating their transplant assessment, and listing them on the national kidney transplant list in a timely manner.

This includes working with the CKD team, Hollins wood House and Ludlow Hospital dialysis units, and our Home Therapies team which include home haemodialysis and peritoneal dialysis patients. This role includes working across site with all the renal departments.

We also deal with living kidney donors and do Phase 1 tests for living kidney donors in our area, and work with the Living Kidney Donor teams at our Transplant Centres

We have patients who are listed for kidney transplant or kidney and pancreas transplant and coordinate their transplant journey and currently deal with seven transplant centres in total.

We also work with patients after they are transferred back to us after a transplant and are involved with their on-going care.

Main duties of the job

•     Timely listing of patients on the cadaveric transplant list.

•     Improving living kidney donor transplantation within the trust and increasing pre-emptive transplantation.

•     Identifying and work up of living kidney donors. 

•     Working closely with the pre dialysis team to improve pre-emptive renal transplant numbers.

•     The post holder will address the specific needs of these patients and work with the transplant teams at the transplant referral centres.

•     Point of contact for transplanted patients within our trust. ? Post transplant care.

They will assist in the development & reviewing of transplant policies and will ensure the efficient and effective utilisation of resources to deliver optimum quality care. They will be responsible for the education, teaching and updating of patients, staff, and junior Doctors within the Renal Service.

The post holder will work flexibly to meet the needs of the service.
