**Internal Vacancy**

To support the Band 7 Senior Complex Discharge Sister to provide a Specialist service, advising and educating all members of the multidisciplinary team in all aspects of discharge management.

Working with partner organizations across the health and social care sector to pro-actively effect safe and timely discharge and admission avoidance.

To provide support to the Frail & Complex Service.

To identify problems and manage with positive solutions using the correct reporting structure. Utilizing autonomous decision making
capability around the discharge/transfer process.

The post will cover a 7-day service including weekends and bank holidays.

To work as part of the complex discharge team to effectively case manage patients, monitor and action as required those on the Fit for Transfer List.

Main duties of the job

An opportunity has arisen to join our team  at The Shrewsbury and Telford Hospital NHS Trust. Applicants must be Registered Nurse with an interest in Complex Discharge  with current experience  in an acute hospital setting. You must be patient centred, and have an understanding of current acute hospital performance indicators. The Complex Discharge role is integral to the successful discharge planning of complex patients within the hospital. To support the Patient Flow and to provide a Specialist service, advising and educating all members of the multidisciplinary team in all aspects of discharge management.

Working with partner organizations across the health and social care sector. 

To identify problems and manage with positive solutions using the correct reporting structure.

Utilizing autonomous decision making capability around the discharge/transfer process.

To work as part of the complex discharge team to effectively case manage patients, monitor and action as needed patients on the Fit for Transfer List.

Ensure patients requiring Pathway 3 are assessed, complete FFA(Fact Finding Assessment) and liaise with Patient and family, ensuring communication is of a high standard. MCA+BI(Mental Capacity Assessment and Best Interest) to be completed where necessary.

Ensure all patients are assessed and appropriate paperwork completed for those eligible/appropriate for Fast-track and CHC(Continuing Health care) funding.

The team is a multidisciplinary team made up of trust staff as well as our partner organisations. We aim to be a highly motivated and cohesive team putting the patient at the centre of all discussions. You will work closely with the ward teams in the completion of assessments of patients needs and ongoing care requirements. The Complex Discharge team can be a fast paced area to work with some challenging and emotional  times but ensuring a safe and  timely patient discharge from the acute setting is very rewarding. We pride ourselves on effective communication and team work to support each other cross site to provide the best service we can.
