This is a fixed term opportunity for 12 months.

The Upper GI Cancer department is looking for an outstanding individual to join the team as a main point of contact in order to support patients throughout their cancer care pathway including those on the new self-managed follow up pathway.

We are looking for an empathetic and IT savvy candidate who is talented, motivated and passionate to work in this new post of Upper GI Cancer Care Navigator.

The post holder needs to be self-driven, highly organised and have excellent communication skills.  Through their knowledge of the clinical pathway requirements, they will enhance the patients experience by ensuring that their pathway is proactively planned and tracked to ensure that patients who are newly diagnosed, on treatment or follow up are supported in a timely and efficient way.  The role will involve liaising with patients, their relatives and clinical and administrative teams within the Trust to coordinate telephone consultations, outpatient appointments, investigations, treatments and multi-disciplinary team discussions.

The post holder will also provide information for the Trusts cancer PTL regarding patient pathways to support the achievement of waiting times standards and ensure effective shared learning from any pathway breaches.

A key part of the role will be to support the upper GI self-managed pathway of care at The Shrewsbury and Telford Hospital NHS Trust (SaTH) by proactively organising the patient’s pathway key events and coordinating rapid re-entry into the system for those people identified as having urgent or specialist needs.  They will be central to the effective communication between patients and the Upper GI Cancer Multi-disciplinary Team (MDT).  The post holder will be based at The Royal Shrewsbury Hospital but cross site travel to The Princess Royal Hospital, Telford will be required.
