At The Shrewsbury and Telford Hospital NHS Trust we are committed to ensuring that as soon as patients are ready to return to their usual place of residence they are supported to do so. In 2018 SaTH2Home was launched to ensure that wherever possible, discharge is facilitated on the day of when a decision is reached that an individual no longer requires acute care.
Following the success of the service as part of Winter 2019, SaTH2Home are piloting a 4 month extension to support more patients where possible with their discharge from the acute setting. This job role will therefore be an exciting secondment opportunity to work alongside the Service Manager to smoothly deliver the service expansion and new pathways. This will involve the post holder managing all administration and co-ordination tasks associated with the work stream.
The post holder will be expected to have key organisational skills as their role will involve co-ordinating the Community Discharge Trackers and working in partnership with the external agencies who deliver SaTH2Home care and support post discharge.
Good communication skills will be pivotal to the role as they will be the key SaTH2Home contact, responsible for updating patients, families, carers, community providers, transport providers and ward staff on discharge plans and arrangements.
