An exciting opportunity has arisen for an individual with passion for providing gold standard care for patients who have been diagnosed with a gynaecological malignancy.

The successful applicant will work within a team of Clinical Nurse Specialists (CNS) and consultants within the gynaecology cancer team with a view to improving the cancer patient experience, as well as supporting the patient with their non-clinical needs.

 

 

Main duties of the job

 

To be a central point of contact for patients with suspected or confirmed gynecological malignancy. Providing non-clinical telephone advice and refer or signpost to other sources of support, guiding them through their personalized care pathway, completing Health Needs Assessments (HNA's), documenting and monitoring aspects of care coordination and service delivery using Somerset Cancer Register, maintaining databases for long term high risk patients to ensure appointments are allocated in a timely manner.  

In addition, facilitating communication between patients and the clinical team, ensuring timely care and support, and helping patients and their families access necessary resources and information to each patient as an individual.

Care Coordination is not one person’s role, job or responsibility. It is the joining up of services, coordination, information and communication between care givers, treatment providers, those living with and beyond 
cancer and their families that creates a seamless experience of care (NHS Improvement, 2011).

Hospital Transformation Programme (HTP)

As part of our long-term vision to improve patient care and service delivery, our Service will relocate to The Royal Shrewsbury Hospital in 2028. This move will unite specialist services in a purpose-built, modern environment, enhancing care and team experience.
