**INTERNAL VACANCY**

The post holder will function as a clinical practitioner within the speciality of Renal Vascular Services. The post holder will ensure a seamless patient pathway for access procedures amongst pre dialysis patients and patients with established renal disease across the SaTH NHS Trust.

 

Main duties of the job

 

The post holder will be responsible for the development and delivery of a high quality service to ensure timely creation and maintenance of vascular access for patients with established renal failure, implementing  and evaluating plans of care for a group patients in accordance with agreed policies and professional guidelines.

The Renal Services is organized by a number of teams, all working together to provide a high quality services for our Kidney Disease population. This particular post works covering duties across the dialysis units at both sites: Shrewsbury Main Unit and Hollinswood House at Telford;  including remote cover to Ludlow Dialysis Unit. The team works closely with Sonography and Radiology teams and liaise with vascular surgeons for vascular access plans.
