We have an exciting opportunity for an experienced neonatal nurse to join our Discharge Team.  

We are based at The Princess Royal Hospital, Telford, in a new purpose-build modern facility.  We are a Level 2 Neonatal Unit within the West Midlands Operational Delivery Neonatal Network.  We have 3 ITU, 3 HDU and 16 Special Care cots. 

We have wonderful facilities within a purpose built unit which includes excellent accommodation for our families.

As a discharge planner you will be working within the MDT team, leading, planning and facilitating neonatal discharges. This will involve teaching parents and staff all aspects of discharge planning including safer sleeping, BLS, nutrition and signs of illness.

You will be responsible for planning holistic care for the babies leaving our care and liaising closely with appropriate community services and allied health professionals to support the babies to thrive in the community.

You will also be required to oversee the discharges of babies going into social care.

The successful candidate needs to be confident to empower parents to take their baby home safely. The candidate also needs be forward thinking, passionate and an excellent communicator.

This role is at band 6 but you will not be expected to take on co-ordinator responsibilities.
