We are working with our community partners to look at ensuring continuity of care in the community and we are looking to progress service provision within Shrewsbury and Telford Hospital (SaTH).
We are seeking an enthusiastic and dynamic practitioner to join our fantastic team. The successful post-holder will be working with the CNS team to help lead the service to ensure our colorectal patients receive high quality care and support.
You will be expected to ensure patient pathways are followed; run nurse led stoma and colorectal clinics; demonstrate advanced diagnostic skills; perform digital rectal examinations and rigid sigmoidoscopy; participate in follow up or rectal bleed clinics and provide care and support for colorectal and stoma patients on the ward areas. You will be expected to share learning with ward staff and your peers to ensure patients receive the best colorectal care.
The successful candidate must have relevant experience with stoma care and have a passion and enthusiasm for caring for colorectal patients. You will be required to work independently with support from the senior CNS team, prioritising your own case load and delegating to the band 6, 5 and HCA as appropriate. This role requires Advanced Communication Skills as you will be liaising with all members of the MDT patients and families. A sound understanding of colorectal procedures is essential for the post holder.
Training would be given for the right candidate, dependent upon experience this may need to be at a lower band or annex U until the competencies are met. Ideally the successful candidate will have the physical examination course and prescribing course, however training can be given to the right candidate.
The post holder will be a role model for professional practice, exercising accountability and providing support and leadership within the colorectal team. You will work closely with the consultant and nursing teams to provide high standards of patient care and achieve national targets.
The post holder will be required to work cross site.
