Job overview

An exciting opportunity has arisen within the Heart Assessment Team for a  Sister position for  37.5 hours per week for a motivated individual with recent cardiology experience. 

You will assist with the development and implementation of all areas of an on-going evidence based programme of care for cardiac patients focusing upon their heart failure and cardiac rehabilitation needs. You must possess excellent communication and interpersonal skills and be able to work well within a multi-disciplinary setting.  You will have proven evidence of being able to study at Level 6/Level 7 academic modules.  As you will be involved in running Nurse Led Clinics there will be a need to undertake the  Independent/Supplementary Nurse Prescribing Course  and the Physical Assessment Course. 

You will need to show commitment and direction  to the team as a whole and actively participate in the teaching, training, support and supervision of all grades of staff including nurses, administrative staff and other health professional students.  You will be required to be receptive to change, dynamic and demonstrate active assistance and support to your Team Manager in the on-going development of the department and service as a whole. 

The successful candidate must be a car driver and hold a valid driving licence as the role may involve cross site covering the Royal Shrewsbury Hospital  and the Princess Royal Hospital.  The base for this vacancy is the Princess Royal Hospital.

Main duties of the job

to provide high standards of evidence based clinical care and advice to patients with heart failure / cardiac rehabilitation needs within the primary/secondary care setting through the assessment, planning, implementation and evaluation of care programmes in accordance with service standards
to contribute to the development of standards, guidelines and protocols for the care of patients with heart failure /cardiac rehabilitation needs in primary /secondary care. 
to provide  support and advice to patients and their carers/family members through a structured programme of education and exercise where appropriate, imparting specialist knowledge relating to their condition and its management, facilitating recovery, self management and future wellbeing.
to be competent  in undertaking physical assessments of patients and to be competent in independent/supplementary prescribing.

The Heart assessment Team  provide a cross site service to patients at both The Royal Shrewsbury Hospital and The Princess Royal Hospital, Telford.
