An exciting opportunity has arisen within the Medicine and Emergency Care Division for a Band 3 Patient Journey Facilitator! We are looking to recruit a Patient Journey Facilitator to be based at The Royal Shrewsbury Hospital to support patients discharge from hospital to home. This role offers a great variety and requires someone who can work under their own initiative and as part of a team. The role is to provide flexible cover to facilitate a patients discharge from hospital. The role requires you to work as a peripatetic Patient Journey Facilitator who will be required to cover a number of wards to support with any absence within the team, or to support another Patient Journey Facilitator due to high level of discharges. You will be required to work flexibly across the wards as required.
Patient Journey facilitators are ward based and focused on delivering the SAFER Red2Green approach to patient care. SAFER Red2Green aims to value patient time and improve flow by reducing delays in a patient’s hospital journey.
Main duties of the job
We are seeking a keen individual with a ‘can-do’ attitude who is able to work alongside the clinical teams to progress the patient’s journey. The successful candidate's will need to be proactive and personable with a strong understanding of SAFER Red2Green and an appreciation for the importance of its principles. To understand the pathway of each individual patient; monitoring their progress and ensuring timely intervention is achieved and to support the discharge plan on the individual patient. You will be required to attend daily board rounds, and identify patients who require the support of social services, Pathway Zero or any other referral to facilitate safe discharge. This is a fast paced demanding role, where no two days are the same. This may include cross site working and occasional weekends. The Patient Journey Facilitator’s role is vital in supporting the wards to ensure patients are discharged in a safe and timely manner.

This is a very busy and proactive role and the Patient Journey Facilitator role is very diverse and work very closely with a range of multi-disciplinary teams to ensure that patient pathway and planned discharge runs smoothly and effective.
