An exciting and rewarding opportunity has arisen to appoint 2 full time Pharmacists.  This is a new 2 year fixed term post for Medicines Optimisation in Care Homes, a joint initiative between Shrewsbury and Telford Hospital NHS Trust (SaTH) and Shropshire Clinical Commissioning Group (SCCG).

The post holder will be required to commit to the Medicines optimisation in care homes training pathway funded by NHS England and provided by CPPE and become an Independent Prescriber.

Building on the previous success and work already undertaken with care homes across the locality you will be part of a small dynamic team that will continue to strengthen the support offered to all care home residents (nursing and residential) and care home staff.

This includes direct access to clinical advice, appropriate on-site assessment, advanced care planning/care planning, medication reviews and optimisation, chronic disease management, post discharge reviews and GP/Nurse care home rounds. This role will involve close working relationships with service users, residential and nursing homes, community services as well as other team members and GP Practices.

You will be supported by the wider pharmacy team and work alongside the Frailty Team based at the Royal Shrewsbury Hospital.
